It has been explained to me that Friendship Manor does a Criminal background check with the State of Illinois, per Illinois Department of Public Health requirements, on all new employees prior to an official offer of employment.

I also acknowledge that I may be required to participate in Fit for Duty and pre-employment drug testing.

I hereby consent to this investigation and authorize Friendship Manor to procure a report on my background as stated above, via State of Illinois.
